Visit Jotform.com, sign up and create your own JotForm account.

Creating a form in JotForm

Two options:

  1. Create a new form by dragging and dropping form elements. Read more on How to create your first form.
  2. If you have existing forms in a PDF format, import them. Read more on How can I import my own PDF. Once imported, edit them so that they're formatted to your requirements.

Config to remember: * The form must contain a hidden field named relationship_id. This field is crucial as it passes the client's ID to the form submission, enabling AppointmentGuru to link it to the client's profile. * Add a short text field to the top of the form with the name relationship_id * In the Properties of the field, click on 'Advanced' and make sure 'Hide field' is on.

Once your forms are created, make sure they're published.
