Only include a JotForm form for specific appointment types

If a specific appointment type, such as an *Initial Consultation*, requires a particular form to be completed, you can include that form in your notifications for that appointment type.

To include a link to a form in the notifications for a specific appointment type:

From the settings on the right in the AppointmentGuru app, go to Integrations, click on Manage next to Jotform.
Click on the linked form that you would like to change and click Settings.
Click Edit next to Form is shared on all appointment types and choose the relevant appointment types.

When including a link to forms in your notifications, your form will only be included if the appointment is of the relevant appointment type.
